MacBeth was killed on a test flight at Castle Bromwich of a HPO/400 aircraft, when the fabric stripped in flight from the lower port bottom wing, and the undercarriage collapsed in the force landing that followed. All onboard were killed: Lt E J Bravery, J B May, C W Offord, A J Winrow, G Greenland and H Simmonds. MacBeth was the only Canadian, the others were all British. Information from CASPIR
